Cracked or Worn Teeth from Clenching: Spotting the Damage Early

Years of clenching and grinding can flatten teeth, chip their edges and leave hairline cracks. The changes are slow, so they are easy to miss. This guide shows what to look for, how worn and cracked teeth are repaired, and how to stop the damage from going further.

Heavy clenching and grinding can leave flattened biting surfaces, chipped front edges, hairline cracks and sensitivity near the gum line. Because enamel does not grow back, the earlier the wear is spotted, the smaller the repair.

Repairs range from bonding to crowns. Protecting what is left, usually with a custom night guard from $230, is what stops the wear from continuing.

Wear from clenching creeps up slowly. One day a front tooth looks a little shorter in a photo. Another day a molar starts twinging on cold water, or a piece breaks off a filling. Each change seems small, but together they tell a clear story about the load your teeth are under.

Possible causes, not a diagnosis

Worn and sensitive teeth can also come from acid erosion, brushing too hard or gum recession. Your dentist can check which explanation fits at a consultation. If biting itself is painful, our guide to tooth pain when biting covers the other common causes.

Why Early Wear Matters

Enamel is the hardest substance in the body, but it cannot regrow. Once the outer layer is worn through, the softer dentine beneath wears faster, so the rate of wear often speeds up. Cracks behave in a similar way: under repeated loading they tend to travel deeper, and a crack that reaches the root usually means the tooth cannot be saved.

Seen early, worn teeth can often be protected with a night guard and small bonded repairs. Left for years, the same teeth may need crowns to rebuild their height. In some cases the bite itself has to be rebuilt.

The goal is not only to repair the teeth that are damaged. It is to stop the same forces from damaging the repair.

How Worn and Cracked Teeth Are Repaired

The right repair depends on how much tooth is left and where the damage is:

  • Bonding: tooth-coloured composite to rebuild small chips and worn edges, usually in one visit
  • Replacing broken fillings, sometimes with a stronger material or a different design
  • Crowns for cracked or heavily worn back teeth, to hold the tooth together and restore its shape. See our page on dental crowns and bridges
  • Veneers to restore the length and shape of worn front teeth, once the grinding is under control. See porcelain veneers
  • Replacing a tooth that cannot be saved, for example with an implant, planned with the grinding habit in mind (see grinding and dental implants)

Fees depend on the repair and are quoted after examination. A consultation at Vera Dental is $40–$50.

Protecting Your Teeth from Further Wear

  • A custom night guard, made from a 3D scan, so the guard rather than your teeth takes the grinding at night (from $230 at Vera Dental)
  • Teeth apart during the day. Lips together, teeth slightly apart and tongue resting on the roof of the mouth
  • Breaks from chewing gum and hard snacks, which keep the jaw muscles working
  • Regular reviews, so new chips or cracks are caught while they are small

For some people with strong, tense jaw muscles, a muscle-relaxing injection may be discussed alongside a night guard. Our teeth grinding and jaw clenching page explains how options are chosen.

Careful Assessment, Clear Options

Assesses grinding and clenching, and performs botulinum toxin injection when it is the chosen option.

Dr. Celeste Ling graduated with a Bachelor of Dental Surgery (Honours) from the University of Manchester and trained across specialty departments at the National Dental Centre Singapore. She is accredited by the Singapore Dental Council to perform botulinum toxin injection.

Many people who clench have been told to "just relax". Dr. Celeste Ling prefers to show you what she can see: the wear on your teeth, how firm your jaw muscles feel, how your bite meets. Then she explains each option, including what it will not do, so you can choose with the full picture.

Protecting the Work You Have Invested In

Dr. Jamie Wong — Founder & Chief Dental Surgeon

Assesses bite, grinding and the load on implants, crowns and bridges.

Dr. Jamie Wong (BDSc Hons, University of Queensland) founded Vera Dental and has over a decade of hands-on experience in implant dentistry. Much of that work is long-term: checking how implant crowns, bridges and natural teeth are holding up years after treatment.

Grinding is one of the things she looks for most closely, because heavy night-time forces are a common reason dental work chips or loosens. Spotting the habit early, and protecting against it with a well-fitted night guard, is far simpler than repairing the damage later.

Frequently Asked Questions

What patients ask us about worn and cracked teeth.

No. Enamel does not regrow once it is worn away. Worn areas can be protected and, where needed, rebuilt with bonding, veneers or crowns. Stopping further wear is the priority.
Common signs are a sharp twinge when biting on something firm, especially when releasing the bite, and sensitivity that comes and goes. Cracks are often too fine to see, so your dentist uses bite tests and X-rays to find them.
Bonding can chip under heavy grinding, which is why repairs are usually paired with a night guard. Your dentist will discuss whether bonding, a crown or another option suits the forces on that tooth.
Often, for back teeth with a crack confined to the crown, because a crown holds the tooth together and stops the crack from spreading. Small cracks may only need monitoring or a bite adjustment.
At Vera Dental, a custom night guard starts from $230. It is made from a 3D scan, and the type and final fee are confirmed at your consultation.
