Teeth Grinding & Jaw Clenching: Protecting Your Teeth, Implants and Crowns

Grinding and clenching quietly wear teeth down, tire the jaw muscles and put extra strain on fillings, crowns and implants. Here is how we assess it at our Tanjong Pagar clinic, and the options for protecting your teeth and the dental work you already have.

Teeth grinding and jaw clenching wear teeth down, strain the jaw muscles and add load to fillings, crowns and implants. At Vera Dental, care starts with a consultation ($40–$50) to see what is happening and why.

For most people the main protection is a custom night guard, from $230, made from a 3D scan. Other options, including a muscle-relaxing injection, are discussed only where they suit you. There are no packages.

Most people who grind do it in their sleep, so they rarely hear it. What they notice is the aftermath: a tight jaw on waking, a dull ache around the temples, a tooth that has become sensitive, or a chip on a crown they had made a few years ago. Daytime clenching is just as common among busy working adults, and often goes unnoticed until the jaw muscles start to ache.

Because Vera Dental focuses on implants, crowns and restorative work, we look at grinding through one particular lens: what it is doing to your teeth and your dental work, and how to protect both. This page explains what we check, the options available, and how a decision is made.

Signs Your Teeth Are Taking Extra Load

Grinding and clenching leave clues that a dentist can usually see before you feel much at all. Any of these is worth mentioning at your next visit:

  • A tight or tired jaw on waking, sometimes with a dull headache at the temples
  • Teeth that look shorter or flatter, or front teeth with small chips along the edges
  • New sensitivity to cold, especially near the gum line
  • Cracked fillings, chipped porcelain or a crown that keeps coming loose
  • An implant crown that feels loose or has needed re-tightening
  • Clicking, locking or aching around the jaw joint in front of the ear
  • A partner who hears grinding at night
  • A scalloped edge on the tongue or a white line along the inside of the cheek

These are possible signs, not a diagnosis

Several of these have other causes. Sensitivity can come from gum recession or decay, and jaw pain can come from the joint itself. Your dentist can check which explanation fits at a consultation.

Grinding, Clenching and Related Concerns

These often overlap, so we look at your teeth, jaw muscles, jaw joint and bite together rather than one at a time.

At Night

Sleep Grinding (Bruxism)

  • Side-to-side grinding or firm clenching during sleep
  • Shows up as flat, shiny wear on the biting surfaces
  • The main reason we recommend a night guard for people with implants or crowns
During the Day

Clenching and Jaw-Muscle Pain

  • Holding the teeth together while working, driving or exercising
  • Can overwork the chewing muscle (masseter), which may enlarge over time (masseter hypertrophy)
  • Often tied to stress, concentration and posture
The Jaw Joint

Clicking, Locking or Joint Pain (TMD)

  • Pain or noise in the joint in front of the ear
  • Difficulty opening wide, or a jaw that catches
  • Assessed carefully, with referral where a specialist is needed
Smile Planning

Gummy Smile, Assessed by Cause

  • More gum than expected shows when you smile
  • Can come from the lip, the gum, the teeth or the jaw
  • The right treatment depends entirely on which one it is

Why Grinding Matters More When You Have Implants or Crowns

A natural tooth sits in a thin cushion called the periodontal ligament. It absorbs some of the force of biting and sends quick feedback to your brain, so you instinctively ease off when you bite something hard.

An implant is fused directly to the bone. There is no ligament, so there is less cushioning and less of that early warning. During heavy night-time grinding, more of the force passes straight into the implant crown, the small screw that holds it, and the bone around it. Over time that can show up as:

  • Chipped or fractured porcelain on an implant crown or bridge
  • A crown or abutment screw that works loose
  • Extra wear on the natural teeth opposite the implant
  • Strain on the bone around the implant, which your dentist monitors on X-rays

None of this means people who grind cannot have implants. It means the habit should be planned for. Before treatment we check for signs of grinding. Afterwards, a night guard and regular reviews help protect the investment. Our pillar guide to dental implants in Singapore covers the treatment itself, and our article on grinding and dental implants goes into the planning in more detail.

Crowns, bridges and veneers face the same risk. Porcelain is strong in normal chewing, but it can chip under repeated grinding. A night guard is often the simplest way to protect it.

How Treatment Is Decided

There are no packages. Whether a treatment suits you, and what it will cost, is decided at a one-to-one consultation before anything is done. The options below are often combined. Most plans start with protecting the teeth.

What Happens at Your Consultation

A grinding consultation is a conversation first and an examination second. Nothing is done on the day unless you choose it.

  1. Your historyWhen the symptoms started, whether anyone has heard you grind, your sleep, stress, and any medicines you take.
  2. Your teeth and dental workWe look for wear facets, cracks and chips, and check fillings, crowns, bridges and implant crowns.
  3. Your jaw muscles and jointWe gently feel the chewing muscles, check how wide you can open, and listen for clicking in the joint.
  4. Your bite and a 3D scanIf a night guard is likely, a 3D scan records your teeth accurately.
  5. Your optionsWe explain what we found, what each option can and cannot do, and the fees. Then you decide.

About Botulinum Toxin Injection for Clenching

Botulinum toxin is a prescription medicine. In small amounts it relaxes the muscle it is injected into. For clenching, it is placed in the chewing muscles to ease how hard they contract.

It may reduce clenching force and jaw-muscle tension. It does not cure grinding, and it does not protect the teeth the way a night guard does, so a night guard usually remains part of the plan. The effect builds gradually over several days, is usually fuller at around two weeks, and is temporary. It typically lasts several months, and this varies from person to person.

At Vera Dental, the injection is performed by Dr. Celeste Ling, who is accredited by the Singapore Dental Council to perform botulinum toxin injection.

Possible side effects

  • Tenderness or bruising where the injection was given
  • Headache
  • Temporary tiredness when chewing firm foods
  • Temporary unevenness of the smile (when used for a gummy smile)

Who should not have it

  • People who are pregnant or breastfeeding
  • People with a neuromuscular condition, such as myasthenia gravis
  • Anyone with an infection at the injection site
  • Anyone allergic to botulinum toxin

Your dentist will also review any medicines you take before recommending it.

How the fee is decided

There are no packages. Whether treatment suits you, the dose and the fee are decided at a one-to-one consultation, before any treatment.

Gummy Smile: Looking at the Cause First

A smile that shows a lot of gum is common, and for many people it is simply part of how they smile. If it bothers you, the first step is to work out why it happens, because each cause is handled differently:

  • The lip lifts higher than average. A muscle-relaxing injection may be discussed. Its effect is temporary and can make the smile slightly uneven for a while.
  • Gum covers more of the tooth than usual. Gum contouring may be considered after an assessment of the gum and bone.
  • The teeth are short or worn, often from grinding. Restoring their length with crowns or veneers can change the balance of the smile.
  • The upper jaw is longer than average. This needs a specialist opinion.

Because short, worn front teeth are a frequent cause, gummy smile assessment often sits alongside smile planning for porcelain veneers or new crowns.

Protecting the Work You Have Invested In

Dr. Jamie Wong — Founder & Chief Dental Surgeon

Assesses bite, grinding and the load on implants, crowns and bridges.

Dr. Jamie Wong (BDSc Hons, University of Queensland) founded Vera Dental and has over a decade of hands-on experience in implant dentistry. Much of that work is long-term: checking how implant crowns, bridges and natural teeth are holding up years after treatment.

Grinding is one of the things she looks for most closely, because heavy night-time forces are a common reason dental work chips or loosens. Spotting the habit early, and protecting against it with a well-fitted night guard, is far simpler than repairing the damage later.

Frequently Asked Questions

What patients ask us about teeth grinding and jaw clenching.

Most people find out from the signs rather than the sound: a tight jaw or headache on waking, teeth that look flatter, chipped edges, new sensitivity, or dental work that keeps chipping. A partner may also hear it. A dentist can usually see grinding wear at a routine examination.
Usually, yes, if you grind. Implants have no natural cushion around them, so heavy grinding forces pass more directly into the crown, screw and surrounding bone. A custom night guard spreads and absorbs that force while you sleep.
A custom night guard at Vera Dental starts from $230. It is made from a 3D scan of your teeth, and your dentist confirms the type and final fee at your consultation.
No. It may reduce clenching force and jaw-muscle tension for several months, but the effect is temporary and it does not protect the teeth. A night guard remains the main protection for your teeth and dental work.
There are no packages. Whether treatment suits you, the dose and the fee are decided at a one-to-one consultation, before any treatment.
Stress is one of the most common factors in both daytime clenching and night-time grinding, along with sleep quality, caffeine, alcohol and some medicines. Noticing when you clench during the day is often the first step in easing it.
